Donor dominance was a term coined in the 1950’s, when it was realized that hair follicles continue to live in their new location. This is the case for the majority of patients, but there are rare occurrences where hair surrounding the transplanted hair begins to shed, giving the appearance of continual thinning. Transplanted hair is usually resistant to the DHT hormone, which causes baldness. Hair at the sides and back of the head are denser and normally resist this hormone. Therefore, these are the areas that the hair is harvested from. There are rare occasions where the donor area begins to thin too. This usually pertains to the family history of hair loss.

If the technician performing the hair transplant is not experienced, it is possible for the hairline to be brought too far forward. If balding continues behind the hairline, it can be unnatural-looking. When there are not enough grafts left in the donor area of the scalp, there is no way to create a fuller head of hair. Because of these anomalies, some facial plastic surgeons prefer to tell their patients that their hair transplant will be”long-lasting”, rather than long lasting.

An experienced plastic surgeon will create a plan for the future, carefully calculating how many grafts are available in the donor area and how much hair loss has already taken place. To stabilize hair loss in the donor area, medications such as finasteride and minoxidil are sometimes used. It is not uncommon for recipients to want a second hair transplant session after 1 year has passed and they are able to see the fullness of their new head of hair. This is not because the first transplant was not successful, but because they desire a thicker head of hair.
